New Care Plan Options in 2024

 IMC is excited to offer a comprehensive wellness care plan to our patients in addition to our Pay-Per Service Plan*

 Our membership plan can be started at any time and covers a full 12 months of care. Patients electing the care plan within 30 days of a paid office visit may deduct that office visit from their membership fee.

Membership Plan Benefits 

·     Free Office Visits (up to 12 visits) in-person or telemedicine (up to $3,000 value)

·      Free Health Coaching - # of visits based on need (up to $1500 value)

·      Free unlimited sauna and redlight therapy visits ($1,200 value)

·      Five (5) Wellness Credits per month which are used to pay for or reduce fees for Infusions, SoftWave Therapy and NES Energy Medicine. Unused credits roll over from month to month.  ($1,500 value)

·      Supplement Discount - 10% discount on all supplements purchased at the practice

·      Free form completion (FMLA, Medical Necessity, Disability, Accommodation)

·      Free referrals and pre-authorizations

·      Free Lab Order Changes

·      Open portal messaging with clinical staff and Providers

·      Dedicated Member Phone Line

·      Free Reimbursify Claim Filing for reimbursement for out-of-network benefits

2024 Membership Plan Fees

 One payment of $2,700 or two payments of $1,400 made six months apart.
